Doctors must adhere to the highest standards of professional conduct and ethics in their interactions with patients, colleagues, and the community.
Doctors agree to comply with all relevant laws, regulations, and policies governing the practice of medicine, including those related to billing, documentation, and professional standards.
The doctor agrees to provide medical services and treatment to the patient in accordance with the standards of care applicable to their specialty and the patient's medical needs.
The doctor agrees to maintain the confidentiality of the patient's medical information in accordance with applicable laws and professional ethics. The patient's medical records and information will not be disclosed to third parties without the patient's consent, except as required by law or in emergency situations.
Both the doctor and the patient agree to treat each other with respect, dignity, and professionalism, fostering a collaborative and trusting relationship.
In the event of a medical emergency, the patient agrees to seek immediate medical attention and to inform the doctor of any emergency situations as soon as possible. The doctor agrees to provide appropriate emergency care or referrals as needed.
Both the doctor and the patient agree to comply with all applicable laws, regulations, and professional standards governing the practice of medicine and the patient's rights and responsibilities.
Any disputes arising under this agreement shall be resolved through mediation or arbitration in accordance with the laws of the jurisdiction where the medical services were provided.
Patients have the right to access their medical records and request copies of their records, subject to applicable laws and regulations.
Patients have the right to continuity of care, including timely access to follow-up care and coordination of services between healthcare providers and facilities.
Patients have the right to receive medical care without discrimination based on race, ethnicity, religion, gender, sexual orientation, disability, or other characteristics.
